Dear Sir or Madam;

My name is Porntip Swangrojn. I am from Thailand. I am currently studying
for my Ph.D. in mathematics education in Science and Mathematics Education
Department, Oregon State University. I am now doing a research on
calculator usage in mathematics education. This research is part of
research course in my department. The purpose of this research is to
reveal decisions which mathematics teachers and mathematics teacher
educators have made on the use of calculators in teaching and testing of
mathematics.

As the amount of calculator availability increases, the reform movement in
mathematics education has suggested that calculators enhance students
learning and should be used broadly in school mathematics. Calculator
facility is becoming an important requirement for mathematics teachers.
However, few studies have been conducted on the impact of calculator usage
on teaching and testing in mathematics. Consequently, this study is
designed in the hope that the knowledge about decision making on the use
of calculators in teaching and testing of mathematics will provide
critical information for the development of mathematics curriculum and
evaluation, staff development for preservice and inservice teachers, and
support systems designed to improve technological implementation.

I am asking for your help. I am particularly desirous of obtaining your
responses because your experience with calculator and mathematics
education will contribute significantly toward solving some of concerns we
face in this area of mathematics education. This instrument has been
tested with a sampling of preservice teachers and graduate students at
Oregon State University, and I have revised it in order to make it
possible for me to obtain all necessary data while requiring a small
amount of your time.

I would appreciate if you would participate and complete the
questionnaire. Other phases of this research cannot be carried out until I
complete analysis of the questionnaire data. I would welcome any comments
that you may have concerning any aspect of the study. Your responses will
be held in strictest confidence. If you agree to participate in this
research project, please go to the following URL to read an inform consent
from and fill out the questionnaire:
                       http:// www.orst.edu/~swangrop
I will be pleased to send you a summary of the questionnaire results if
you desire. Thank you for your cooperation.

Sincerely,
Porntip Swangrojn
